DESCRIPTION:You are invited to the Joint County Transition Conference and Resource Fair\, brought to you by the Alameda County Developmental Disabilities Council\, the Developmental Disabilities Council of Contra Costa County\, and community partners. This educational conference is for students and individuals of all ages with disabilities\, their families\, and their circle of support\, to learn about transitioning to adult services and building independence. Presentations will focus on early teens to the post transition years (22 and up). \n  \nEstán invitados a la Conferencia de Transición y Feria de Recursos Conjunta del Condado de Alameda y Contra Costa\, organizada por el Consejo de Discapacidades del Desarrollo del Condado de Alameda\, el Consejo de Discapacidades del Desarrollo del Condado de Contra Costa y socios comunitarios. Esta conferencia educativa está dirigida hacia estudiantes e individuos de todas edades con discapacidades\, sus familias y su círculo de apoyo\, para aprender sobre la transición a los servicios para adultos y como construir la independencia.
